*NEW WHOLESALE CUSTOMERS*
1) For first time wholesale orders please fill in the form below including Tax ID# or Fed. EIN#.
2) Once you submit the contact form, you will automatically be directed to our wholesale shopping page where you may place your order at any time.
3) Minimum wholesale order is $75. Your order will ship via USPS Priority Mail for $8.50.
